Season survey Drawing

Thanks so much taking the time to fill out this short survey! We use this information to help us better plan our season. 

All forms submitted prior to September 15, 2021 will be entered in for a drawing to win a $100 restaurant gift card for the person filling out the survey and also $500 credit for their organization to use toward any competition this season! 

This form may be filled out by any owner, employee or coach of any organization that MAY compete with us this season.  There is no limit to the number of entries for any organization. All official coaches and employees of any organization may enter. 

Four(4) names will be drawn. The drawing will be confidential and the winners will be notified directly via email. The results will not be publicly posted. The survey answers will confidential as well. 

On the  "# teams and athletes you plan to bring" to each event questions just enter the two numbers. For example if you have 3 teams with a total of 45 athletes just enter 3/45. If you are not bringing any to that event just enter 0. If you are unsure or don't know exactly how many you have enter in your best guess. 

If you are already registered for a competition you still need to fill this out to qualify for the drawing. Remember all coaches and employees of any organization planning on competing with us can enter.
